Copy of Current Passport or Birth Certificate
Purpose: Age Verification

Original Scorecards (for players without an index)
Purpose: Player Skill Level
Send by: Mail Only
*NOTE: Scorecards must be originals (NO COPIES) and must have three signatures (player, witness, PGA Professional).  iTOUR Players must submit six 9 hole scores and eTOUR Players must submit five18 hole scores.  

Copy of Most Recent Report Card or Unofficial Transcript 
Purpose: Academic Eligibility

*NOTE: Players must maintain the NCAA minimum 2.0 or B/S average in order to compete in NCPGA Junior Tour events.
